STEP 1 REGISTER ON FAMILY ID

If student was already registered in Family ID for this season's sport, band, color guard or band, please don't re-register but use existing Family ID registration to upload forms in Step 2-5. Parent must register student on FamilyID for every Sport, Drama, Color Guard, or Band group every season.

STEP 2 PRE-PARTICIPATION CONCUSSIONFORM (UPLOAD TO FAMILYID)

Parent must complete form-sign and have student co-sign. Upload the form to the FamilyID Registration. Parent must complete and upload the form to Family ID for every Sport, Color Guard or Band group every season.

STEP 3 PARENT CONCUSSION VIDEO CERTIFICATE (UPLOAD TO FAMILYID)

Parent and student each have their own nfhslearn account. They order and watch free concussion course video and upload into Family ID the 2 certificates (one for a parent and one for a student). Do not have to do if already uploaded a Parent Concussion Certificate for school year (August through June).

STEP 5 COPY OF CURRENT PHYSICAL (UPLOAD TO FAMILYID)

Athletic Office must have a physical less than 13 months old from exam date. Upload physical to FamilyID.
